Specialized Care for Kids, Right at Home

With a team of board-certified pediatric professionals providing 24/7 remote care, our Center for Advanced Care at Home offers comprehensive support to ensure a smooth transition from the hospital to home, while maintaining the highest level of care for your child.

Your child will remain in the program until they reach specific health goals. Once these goals are met, your child will transition to the supervision of their primary and specialty care team, ensuring a seamless continuation of care and support.

How it Works

Your child’s care team includes:

  • Nurses: Your child will receive virtual visits from our nursing team to monitor their health and provide guidance and support. The frequency of nursing visits is determined by your child’s medical condition.
  • Pediatric specialists: Our health care providers will conduct virtual visits to assess your child’s progress and address any medical concerns.
  • Advanced care technicians: We’re available day and night to connect you with the help you need, whether it’s from a nurse, a specialist, or tech support.

Our in-home health care services include:

  • Remote patient monitoring: We monitor vital signs, ensuring that any changes in health are promptly addressed.
  • On-demand support: Use the ÎÚÑ»ÊÓÆµ Children’s MyChart app to connect directly with your child’s care team, access on-demand and scheduled virtual visits, and get your child’s medical information.
  • Medical equipment: We provide medical equipment and services tailored to your child’s needs.

Who Is Eligible

Our program is designed for kids who:

  • Are transitioning from a hospital stay and need continued clinical oversight
  • Are medically stable and ready to continue care at home
  • Require ongoing monitoring, feeding support, or chronic care management

Our Team

Jennifer Liedel, MD

Medical Director

Dr. Liedel is a board-certified critical care physician and has been providing expert critical care to children since 2004. She has conducted research on several topics in neonatology and has lectured regionally and nationally on topics such as feeding issues in the Neonatal Intensive Care Unit (NICU), breast milk and inflammation in newborns, and the use of extracorporeal membrane oxygenation (ECMO) in the NICU.

Christopher Beaty, MSN, RN

VP, Nursing/Operational Innovation

Christopher Beaty leads clinical innovation across our system. He has spearheaded strategic projects including a multi-phase NICU expansion, the establishment of an outpatient perinatal testing center and most recently the formulation of an operating plan for the nation’s first specialized support program for children transitioning from hospital to home.

Collin Miller, DNP, CPNP-AC

Director of Nursing

Collin leads virtual care initiatives, leveraging his extensive experience and expertise in pediatrics. He has supported the development of the Clinical Pathways department and the launch of over 15 clinical pathways, as well as numerous improvement initiatives including trauma room redesign, electronic trauma documentation implementation, and triage nurse education.
